Connecting care navigation and ongoing support across the healthcare continuum

•2 min read

Keeping the patient story connected

A consultation can end while important questions remain. A report may need to reach another professional. Further examinations may need to be arranged. The patient may be unsure who to contact when a question arises or how the different parts of their care fit together.

Malm focuses on those connections. Its approach follows the person across appointments and services, with attention to the information and support needed at each stage.

Proposed quotation

"The patient should not have to be the only person holding the whole story together. Our role is to help keep that story connected, so that the next step in care makes sense in the context of everything that came before it."

Pedro Stark, CEO of CW1

Five capabilities supporting continuity

The CW1 healthcare continuum brings together five connected capabilities:

Patient context. Understand the person's history, available documents, needs and goals.

Care navigation. Help the patient understand the next step and navigate the appropriate care pathway.

Coordination. Connect the professionals, information and follow-up arrangements relevant to the patient's care.

Follow-up. Support the patient between appointments and help keep agreed next steps in view.

Longitudinal insight. Review information over time to inform subsequent questions, discussions and case review.

Context for the next decision

A new report or concern needs to be considered alongside what has already happened. Longitudinal insight brings that perspective to the care journey by connecting current information with earlier assessments and follow-up.

Within Malm, this perspective supports document review and care navigation. Technology helps organise information and communication, while clinical assessment and treatment decisions remain with the responsible healthcare professionals.

Continuity in practice

Consider a patient who leaves a specialist consultation with a report, further examinations to arrange and a follow-up visit ahead. They may still need help organising the paperwork, understanding which questions to raise with which professional and keeping track of the next steps.

Within the Malm model, support can include reviewing the available documents, helping the patient navigate the agreed pathway and following up between appointments. When a question requires a clinical decision, it belongs with the appropriate clinician. The aim is to help the patient understand where they are in their care journey and what needs to happen next.

Clear responsibilities across the care network

Continuity depends on how people work together. Participating organisations need clear roles, agreed points of contact and appropriate arrangements for accessing patient information. Patient consent and preferences, alongside each clinician's professional responsibilities, shape how that coordination is organised.

CW1's approach is designed to work alongside existing healthcare professionals and organisations. For prospective partners, the starting point is a defined patient group and a clear coordination need, with the scope of participation and follow-up arrangements agreed around that use case.
